Introduction:

There are various advantages associated with the support system in a health care center; the advantages will be realized if a proper system is put in place, the system should be designed in such a way that supports the needs of the users such that it is easy to record new entries, search record, edit record and also easy to navigate.

Some of the advantages associated with the system include storage of data, increasing efficiency.

Contents of the data base:

Thre proposed electronic health record will contain various tables and forms, there will be a table that will record patient information, an encounter table and physician table, all these tables must however be accompanied by a form which will help in entering data, the contents are discussed below

o Encounter form and table

The encounter form will be a form that will aim at recording the encounter type, complaints by patients, diagnosis and also an image shopping various disorders, the form will store this information in the encounter table where information can easily be retrieved by either checking all of the entries or through a query.

o Patient form and table

There will also be a patient information form that will help in entering data regarding the names of patients, address, payment method, patient id and other important information, after completing the form the entry is saved in the patient table.

After preparing the encounter and patient form there is a need to prepare another form that will include the patient form and the encounter form as sub form, this will help in ease of entering data using the same window in a computer, the two will take the following form:

From the above diagram the first form is the patient information form and the second is the encounter form which is a sub form in this window, there are also other commands in the above diagram that helps to save, exit or delete a record.

o Physician form and table

The physician form is also an important part in this data base, this is because it is used to record data regarding the physicians and also their specialty, this table and form is modified to record the names of the physician, specialty and also can be used to record salary and other important information.

o Switch board

The switch board is also included in this data base, it is used for ease access to the various forms in the data base, the switch board contains commands such as open main patient form which contains the patient form and the encounter form as a sub form, it makes it easy to open, edit and close the application, the following diagram shows the switch board:

o Combo box

The combo box is also included in this data base, it is used to retrieve data from a reference table created in the data base, and the following diagram shows the combo box:

o SQL

An SQL is also important in retrieving data, it makes it easier to find records in the table, and it is constructed by specifying its function in design and therefore makes it easier to use the data base especially when searching for data and the following diagram shows the SQL:

Advantages and disadvantages of EHR

Storage of data:

The system is important in the storage of information; this information can easily be stored, edited and retrieved.

Efficiency:

This system will also promote efficiency in the workplace; this is due to ease of communication and also retrieving and storage of data.

Better diagnosis and services:

Diagnosis is made easier because the physicians can easily retrieve data on patient’s medical history.

Disadvantages:

The system has a disadvantage in that it is expensive to install such a system with the center being required to purchase computers to serve this purpose, it may also require training of employees on how to use the data base.
