how to write a request letter when i lost my school i.d?
i already searched the net but i cant find the right format to write a request letter. i need it badly it must contain that i am requesting the school or the dean to give me this “slip” telling that i should be given an affidavit of lost because i lost my i.d,.. pls help me! pls.. or if ever write me an example of the rquest letter. containing why i lost it when i lost it and to give me the slip. tnx

use a full-block style of business letter…
maybe in your content will goes like this..
greetings! i(state your name year and sec. course ect.) is requesting for a slip for my school i.d. was lost last(date when you lost it) for the following reasons:
1…………..
2…………..
(state your reasons in number or bullet form)
