An affidavit is simply a written statement that can contain just about any information you desire, but the whole idea of an affidavit is that you declare under oath that the facts and/or information in that affidavit are true and complete to the best of your knowledge and belief. That’s why you have to sign an affidavit in front of a notary public — the notary will verify you are who you say you are and will administer the oath.

When the person failed to or did not, or stopped paying child support, that person was in contempt of court, as the child support was court ordered. The court is postponing for 12 months, the contempt charge to give time to pay up all child support owed. (feel lucky they can put you in jail) . If the child support is not paid up, then the contempt charge will kick in and off you go to jail. So, you have two charges, failure to pay child support and contempt of court. Pay up, and the court will probably drop the contempt charge, otherwise, if the support is not paid, a warrant will be issued , and you will have no defense. Pay up now or go to jail.

It means they want you (the parent) to give your underage child permission to participate. You must sign it and date it.
P.S., about that guy’s answer below, you are not going to have to get it notarized….it’s a contest, not a tax audit.
