In a letter of recommendation, put the date at the top, skip several lines and then you address it (to the name the student gives you) or if none is given then, “To whom it may concern:”

works well. (Above this, type RE: (name of student), then skip another line.)

Begin the letter by writing about how long you have known this person, how you met, and your positive impressions, etc. about him or her. Make sure your comments show the characteristics that will impress the medical school: compassion, intelligent, committed, dedicated, etc.

Examples of letter can be found in books also.
