It’s in the fine tradition (/snort) of referring poetically to women’s sexual charms as food, usually sweet food — “cherry”, “candy”, “cake”. Now “milkshake”.

i thought it was in reference to her style of dancing – like shaking her booty maybe? cause later in the song she says something like “watch, i’ll show you how” or maybe its her talent for BJs. I also think it’s a play on words, like if she were a little girl with a a lemonade stand, her lemonade was the best around, so it would bring all the boys to her front yard haha
