Try email if you aren’t comfortable enough to call them up on the phone. That way you can ease into the introduction without an awkward “hi, remember me?” phone call.

I would call, it’s more personal. You wouldn’t be asking for a recommendation if you hadn’t been a good student, so chances are he’ll remember you quickly.
