Does anyone know if international medical graduates can enter a combined residency program?

I want to know what kind of restrictions apply for international medical graduates when seeking a residency, especially if they can enter a combined residency, can make chief resident, etc. Also, if clinical experience in the US is mandatory

If you plan to attend an international medical school you need to visit the Educational Commission for Foreign Medical Graduates: http://ecfmg.org/

To answer your question: Some international schools are accepted by all 50 state medical boards and there is no distinction made between a US graduate and an IMG. There are some international medical schools that are not acceptable in a few states and there are some schools that are not acceptable in any state. If your reference to international medical schools is referring to the medical schools located in the Caribbean, all of these schools do their clinical rotations in the US.
