If you’re in Brooklyn you can probably just go to the school and fill out an application there. If you’re in Manhattan you can go to the CUNY info center on 42nd Street in the Grace Building across from Bryant Park and fill out an app there.

THis is what I have. Doesn’t look like the paper copy that you want. Have you considered reporting to the Admissions Office and asking for it in person? I mean, it is a community college, you will be going there daily, it can’t be that far.

It sounds like what you want is a paper copy of the application. I don’t see how I can generate that.
