a. True (depending on your definition of “small”, if the children are old enough to communicate, then they are old enough to understand simple rules)

c. False (a base set of rules should be followed at all times, individual activities may have additional rules)

d. False (not at all. In my classes and those of my teachers, we have students come up with their own rules for the classroom)

Forget about rules in case of small children, as in the case of Grammar rules. You use correct grammar, and you are not very conscious of rules. Teach them the use of rules, and apply the rules and practice them to demonstrate. At a later stage, you can perhaps teach the children the utility of rules, without ever mentioning them. Teach the use and utility, not rules. All the options you gave are in adequate.
