The first is not correct, though you could change it to

Reception will be immediately following. Its more awkward though and I would use the second or “Reception to follow”

The second is correct, but cumbersome. The customary way of stating this is simply to say “Reception to follow” or “Reception will follow”. The immediately is generally implied.

Maybe you can add it to the previous sentence like this :

< ur schedule >…… followed by reception immediately/thereafter.

First one is not right at all.
