It should read “I have much greater difficulty in making decisions than I used to.” The words “than I used to” should make more sense to you.

I’m going to guess that you are not a native English speaker. “Used to” is an idiomatic expression in English, referring to an activity that happened repeatedly, continually, or habitually in the past. E.g. “I used to live in town, but then I moved to the country.” This means that, in the past, and for an extended period, I lived in town, but I no longer do. (Even without the “…but now…” phrase, “used to” always refers to an activity or state that is no longer true.)

In the sentence you gave: “I have much greater difficulty in making decisions than I used to” means that, in the past, for an extended time, I made decisions, and now I have more diffulty making those decisions than I had when I made them in the past.

It means I am having difficulty now in making decisions on certain matters. Previuosly, I did not have any difficulty in making decision on the same matters.

“that” should be “than”

In the past, you found it difficult to make a decision. You now find it even more difficult to make a decision. Actually, “much greater” = “quite a bit more difficult” or “a lot more difficult”.
