I would guess that whatever your father’s family name is also your family name and what ever you use as your first (or given) name should stay that way. Do you have a birth certificate or passport listing your name? Seems to me, you can just about name yourself here. Otherwise I think you technically should be listed as “Ishaq Muhammad ” [like “John Doe”].

Surname=Muhammad and Given Name=Ishaq. Or If Muhammad is a name of respect and Ishaq is your only other name, why not go by Muhammad Ishaq Ishaq?

Reminds me of when the immigrants came to America in the earlier 1900’s and whatever name they gave to the officials as they were getting off the boat became their name for good.

Is it possible to ask a religious clergy from your faith what they recommend? I don’t know how many read Yahoo Answers so you will have to go to your place of worship.

From what you added on to your question, I would say you would tell them your first name is Muhammad and surname Ishaq. I would think Khan is some sort of title.

I remember a similar conversation with a friend from many years ago. IF memory serves me correctly..the ‘Khan’ part would be possibly considered as surname. I can’t recall the ‘why’ of his explanation, just that it was his explanation.

I would suggest you call the University Registrar’s office and explain your conundrum as you have here. They should be able to help you with filling it out. Just make sure you make note the person’s name you are speaking with in case questions arise in the future.

I’m a little confused by what you are asking. Surname refers to your last name and given name refers to your first name. I’m not sure what you mean by family name, but most people consider their last name/surname to be their family name.

As in Ethiopia, may I assume Mohammed is your father’s name? If so, you would use that at your “surname” (dirivitive of “sire’s name” ie you father’s name). I hope that helps.

what do you have?

edit:

oh, are you from pakistan? i know of pakis that do that. anyway, what country do you live in. because when you come to america you can change it. my uncle got his visa to come here when he was in spain and when he came here is changed the spelling, because it was spelled in french, and two letters were out of order, so i am sure you can change it
