Then, you need to call the physicians (and surgeons?) who had personal knowledge of your medical condition, and ask their offices to send a fax IMMEDIATELY to the director of admissions (and appeals person, if any) at UCSD. Give them permission to provide details about your medical condition, and be sure to provide them with the UCSD admissions fax number, and the names of the persons to whom the fax should be addressed. (They may require written permission to release information to UCSD. Ask what you can do to expedite this process.)
Then, follow up with an email — and a mailed hard copy — to the director of admissions (and appeals person) that explains your medical condition, and how it impacted your academic work during the last semester of your senior year. Make this letter professional, clear, concise, and inclusive of sufficient detail. Watch your tone — do not plead or whine.

Has this already happened to you? If not, and it’s been some time since they’ve received your final transcript, I wouldn’t worry about it. If they have, I’d get that legitimate reason to them in an apology letter as soon as possible. If you’re just sending off your transcript, I’d attach a note explaining the difference in last year’s grades.
