My recommeder for grad school is MIA, can I get a replacement although her name is not on my application?
I think my recommender had a family emergency or something because he’s always reliable.

Absolutely, get a replacement. Something like that can happen to anyone, and I’ve often had to substitute references when one became unavailable, for whatever reason. The schools should certainly understand this.

Also, contact the school(s) you’re applying to, especially if you’ve already sent in most of the application. Ask them if they’d prefer you get someone else to write a recommendation for you, and who would be acceptable. For example, if it’s your guidance counselor who is absent, should you have a teacher (whom you know well) write one for you, or should you ask a different counselor (who has your record but doesn’t know you at all) to write it? Or should you send both?
