My college is requires proof of vaccinations… but I’m confused.?

I have to fill out this sheet for college that gives the dates of vaccines that were given to me. But its confusing… it clearly says at the top of the page that the MMR and Menningitis vaccines are required. The MMR has to show two does after the age of 12 months… okay, I can do that. The Menningitis one is proof of one dose (I’ll have to get that one).

But then there are things on the sheet that are more sketchy. There is a slot of Polio vaccines given (4 doses) and DPT (4 doses)… but it doesn’t say when these vaccines needed to have been given. I was given the Polio vaccine 4 times when I was a baby… is this what they want? And I was given the DPT vaccine a total of 4 times when I was a baby, plus a booster prior to starting school when I was 5. The DPT was not required in my state to enter middle school… and we decided not to have me get it because I had an adverse reaction the last time. So which dates are they wanting from this… the ones from when I was a baby?

They are requesting the dates of the shots regardless, when they were given. If you got them in infancy, then document that date.I would also note somewhere docuementing the adverse reaction to the last DPT. So this is part of your student health records.

Your immunizations sound up to date (have you had the Hepatitis A and B vaccinations?). What should you do?

1. Contact a counselor or admissions representative from the college and ask if you can attach your immunization record to the application for admission.

2. Take the application to your physician’s office and ask them to complete the information.
