You should put it under any type of supplemental information listing they provide. Include any info like ISBN, publisher, what page, etc. You can also put it down in extracurricular activities. If this book is actually sold in bookstores, you may even include it as work. Unless you’re planning on becoming a writer or you think the poem will have a direct relationship to what you want to be, probably don’t need to include the actual poem.

You should send in your AP scores to each college. Information on how to should be in those white booklets they give you. They have to send it in, not you, in order to make it official.

Yes, you should put it in. It is an academic award regardless of who gave it.

i’d submit the poem on a separate page…..if i’m not mistaken, your test scores and transcripts need to be sent in a sealed envelope by the originators directly to the colleges, meaning your previous schools must send transcripts and whoever graded your tests must send final score letters (at least that’s how it worked 20 years ago). you can always call the admissions office at the college to find out for sure(they get these kinds of questions all the time)…..and YES, list that in the academic reward section.

one other thing….in case money is an issue for you, call the financial aid office for information. ask for all available grants and scholarships you’re eligible for. they are extremely resourceful. they have all the applications right there on hand.

God bless.
