Yes a lot of schools offer a Bachelor/Master’s 5 year program in various degrees.

It depends upon the subject AND the type of college you’re attending; but as a general rule, the answer is “no”. Grad school has different entrance requirements and there’s no way to know if a person who can earn a BS could do well on an MS (or MA) curriculum. It would be horribly unfair to a student to “lead them on” and then discover that they cannot handle grad school-level work…which is substantially harder.
Some Liberal Arts colleges, however, do allow you to begin MA work once you’ve completed the undergrad curricula even if you haven’t officially graduated yet. The big research universities generally do not do this.
