It is called a reverse complement.

Walking on broken glass is obviously unpleasant. You cut your feet up and get shards of glass stuck in your skin, thus making it painful to walk for a couple of days.

When someone says that to you, it means that they dislike being around you. They find it hard to concentrate after that, because they are so stuck on the sound of your voice grating against their sound nerves.

Similar comments include “You are as pleasant as rancid garbage”, “You look about as lovely as a hairless cat” and “You smell so wonderful that every time I am near you, I want to barf.”

Pain aside, it suggests that the person it’s being said about is touchy or reactionary, and people have to be super careful about what they say and how they behave around that person. Nobody can relax and be themselves.
