I believe you may be confusing one vulgar saying for a portion of another non-vulgar statement.

You have your HEART on your sleeve, means one is wearing their emotions on the outside and showing openly their feelings.

As you have written the statement here, you need to replace the word HEART with another word which sounds a bit like heart, but gives the statement completely vulgar meaning. I’ll not write the statement out, but the meaning is that one is excited or arroused about a particular situation.

In my time, this statement was only used by vulgar men about a particular situation. These days, it seems, that this phrased is used by just about anyone under 30 for portraying exitement from anything from food to movies to sports to you-name-it.

My suggestion is that you do not use this phrase and not perpetuate vulgar language.
