Yes, it’s a metaphor meaning the person felt like a part of their spirit had died, but people do use this all time in a sarcastic way. So, depending on context it could be meant as funny or serious.

They are saying it is something very disappointing, or sometimes sad.

Ex: When I found out the event was sold out, I died a little inside.

They are saying it because they were let down, dissapointed to the degree that it felt like a little part of them died inside…They are sad….disheartened…
