Dear Dean Whosis,
My school identification is lost and need your help to get a new card issued to me.
As far as I know, my ID was given to the Security Center for safekeeping after I was mistakenly accused of littering by _________ (name of utility guy or just write “utility worker”). The worker (or whoever took it from you) confiscated my ID but when I went to retrieve it, the Security Center staff told me that they do not have it. They have advised me that I must have a letter of permission from you in order to get a new document.
Without my ID, I am unable to (fill in your difficulties here, such as “I am unable to check books out of the library” or “resident services are unavailable to me” or both). I would very much appreciate your giving me the needed letter as soon as possible.
Thank you very much for your attention to this matter.
Sincerely,
Suzie Student (your name, of course!)
