how do you write an annotated bibliography?
Does not seem simple.. please help!

First make up your bibliography, using whatever style your teacher requires (MPA, APA, Chicago, whatever). Then add your own notes.
Usually your teacher will want notes that talk about the quality or reliability of the book you’re annotating, rather than a summary of what it covers (or it’s plot if it’s fiction), but sometimes a summary, to prove you read it, may be exactly what they’re looking for, so make sure you find out what is expected.
You might want to say things like, “this book nicely summarizes the conflicts that caused the Civil War, though the author’s conclusion that Britney Spears has no business being a mother seems slightly out of place.”
