i was only a foot away from the door when BAM! i was hit full in the face
how should it be?
I was only a foot away from the door, when…BAM! I was hit full
where should the punctuation go? should i out a comma there? should the […] be there?

There isn’t a single answer to this. Different authors might do it different ways. My way would be this:
“I was only a foot away from the door when — BAM! — I was hit full in the face.”
You could also use commas instead of dashes.
I would not use suspension points [ . . .]. They tend to suggest time passing, and that’s not at all what you mean.
