Do I need a comma after the word time?

Absolutely.

If the sentence were rearranged, it would read:

“I could hear my cousin squealing and my sister bawling, this time.” “This time…..” defines ‘when’ you heard them. It could be removed and the main sentence structure holds true to sentence form. Do the comma and get it right.
