Full saying is “Butter wouldn’t melt in his/her mouth” and it is usually sarcastic and means that the person is whiter than white and wouldn’t ever say or do the wrong thing

It’s a very old phrase meaning someone’s personality is cold, prim and proper, so they wouldn’t even have the warmth to melt butter. These days, though, if you hear it said about someone it usually means they seem so innocent and pure, but it’s meant sarcastically. It tends to be used after a child has been naughty or someone has been rude or obnoxious but at that time in an angelic state such as playing very sweetly or sleeping etc I have never found it to be an offensive term as everyone gets cross or irritable at some point and all children miss behave at some time but like I said used after an incident

I’ve heard it said about someone’s attitude, as in being really cold and indifferent. Also I’d seen this in a blog, where I presumed the author meant it in the same way. I can’t be really sure without the context.
