It comes indirectly from “whack”, meaning a blow. And “wacky” meant acting as if you’d been hit in the head. In the 18th century, “whack” started being used in thieve’s lingo, meaning a share of the loot, and from that it meant to mean an expectation. So, if something was “out of whack”, it was outside of the agreement or expectation or alignment.

from Webster; whack-a smart or resounding blow;the sound of such a blow;portion/share;condition/state…the machine is out of whack;an opportunity or attempt to do something;chance; a single action to do something at once…I am going to make 3 pies at a whack.
The word has many meanings and each is used for different things. Out of whack just means not feeling yourself or something not working the way it is supposed to.
