Where does the expression “Fair to Middleton” come from?

My granny always used to say it. She had lots of other daft expressions. She did not come from Middleton but it was somewhere nearby.

‘Fair to middling’ means mediocre or moderately good. I’ve given you a reference for you to decide where it comes from. It dates back to around 1837.
