It means that before you know it, you’re old. Unfortunately, by the time you get smart, you’ve already made lots of mistakes. It’s a regretful proverb,often spoken by older people who wish they could be young again and still keep the smarts they have earned through experience.

It depends on ones definition of “old,” I guess… I mean some people think 30 is old; I know I did when I was too young to know better! ;-}
So what I think the phrase means is that we become infirm in some ways too soon in life, while we also get “smart” from life’s experiences and teachings too late to act on them in good health and/or good faith.
True for some, I’m sure, but not all.
