for example when someone says ” let me know if you are home” does it mean let me know if you get home? or if you are home right now, just tell me you are home?

‘Let me know when you get home’ means the person is not home yet, but will let know when he gets home.

If you want to confirm if someone is at home or not – perhaps in an e-mail – ‘let me know if you are at home (or not)’ is better.
