“From ancient grudge break to new mutiny…” A long standing hatred has been arisen by the fight between the relatives of the Montague’s and Capulet’s. “Where civil blood makes civil hands unclean.” The civilians of Verona have shed each others blood, not necessarily to death, but the blood they have shed is now on their hands and this makes their hands contaminated.

ancient grudge is rivalry between families.. it breaks into mutiny- thats foreshadowing that something bad is going to happen.. where civil blood makes civil hands unclean- people are going to die and it will make people guilty.

this is all part of the intro.. he’s basically saying what is going to happen in the play before it actually happens

From that old-time grudge that the families had, a new fight has broken out……. and the blood of the people on their hands makes them guilty…

It means, in essence that the Montagues and the Capulets can blame the deaths of their dear children on themselves.
