A “belfry” is the bell tower on top of an old church. Sometimes a belfry would actually be used by bats as a nesting place.

At sunrise or sunset, you could see dozens of bats flapping wildly around the belfry, looking totally disorganized as they tried to land or take off without colliding with each other. So, someone whose thoughts go wildly in all directions, is said to have “bats in the belfry”.
