Back to the payment plan form, you only need to send it in if you are planning to do a payment plan. If on the registration and payment form, you check the payment plan box on page 2, (and select an option under that) then you need to fill out the payment plan form and send it in with the second form (fax or mail).
I hope this makes sense. It is a little confusing doing all of this school paperwork. Good luck!

That’s clearly the second one (which has those words in large blue letters across the top).
And the form has a fax number on it, which answers your other question.
Seems like you need to read the forms a bit more carefully.
