OK I see your confusion. The first form you mention is a payment plan form. I’ll get back to that shortly, you may or may not need to fill it out. You do need to fill out the second form, which is the registration and payment form, no matter what. That needs to be either faxed or mailed in with your payment. That means if you are faxing it, you need to use a credit card and fill out the info at the bottom of the second page of the form. If you are mailing it, you can enclose a check with the form. This is the only form you need to fill out if you are making your payment in full.

Back to the payment plan form, you only need to send it in if you are planning to do a payment plan. If on the registration and payment form, you check the payment plan box on page 2, (and select an option under that) then you need to fill out the payment plan form and send it in with the second form (fax or mail).

That’s clearly the second one (which has those words in large blue letters across the top).

And the form has a fax number on it, which answers your other question.

Seems like you need to read the forms a bit more carefully.
