Stars were placed on the doors of the nicest changing rooms in the early days of Vaudeville. They were reserved for the headliners that brought in the crowds. If you were a moneymaker for the theater you were a “star” instead of a bathroom.

I’d say Lilly Langtree back in the west might have been the first.

Actors were a very rare breed and I think they were called ‘stars’ because of their status and popularity. There were many ‘stars’ early on depending on the genre. Like comedy I always think of Laurel and Hardy; silent films I think of Charlie Chaplin; etc.
