There are two major types of cholinesterase. True or acetylcholinesterase is found in red blood cells (RBC) and nerve tissue. It is responsible for the prompt hydrolysis of acetylcholine released at the nerve endings to mediate transmission or the neural impulse across the synapse. Pseudo-cholinesterase is found in serum and plasma as well as liver, pancreas, and the white matter or the brain. Its physiologic role remains unclear (1). The measurement of the activity of both enzymes is valuable in the evaluation of exposure to organophosphate pesticides if proper care is given to specimen collection, analysis and interpretation of results.
