So, this is what I’d do. I’d go with a brief profile something like this:

“Having acquired x number of years experience providing customer service (or whatever you did), I seek new challenges through an intern or entry position in Project Management”

Then go on with the rest of your resume — but look for ways to demonstrate you have those excellent qualities. For example, where did you demonstrate leadership skills? If this was in a job situation, you might say, “Demonstrated leadership by organizing and chairing a committee to addess the ….”

The same with all of your qualities. Where did you work independently? Say something like,”Worked independently to accomplish….” or “Participated as a team member on multiple projects…

See what I mean? Show them, not tell them.

If you are including a cover letter, you could also elaborate on your organizational skills, etc., in the cover letter.
