Informal assessment is experience-based and doesn’t have normative data.
Formal assessment is score-based and entails using a pre-determined format which has normative data.
You aren’t going to find what you are asking for; to my knowledge, it doesn’t exist, unless it’s a bedside swallowing assessment. There are plenty of papers in the literature which will give normative data about the effectiveness of a bedside exam.
