The mouth of a river is the place where the river ends. This is frequently at a lake or ocean. A river mouth is really just one outlet for a river.

A delta is a specific type of river ending. In a delta, the river fans out to form many outlets.Some of these outlets could be old and no longer true river outlets. Think of the Mississippi delta in New Orleans.
