“is” should be “are” because you have a compound subject (a job directory and a list). It would sound better also if it was worded like this: A job directory and a list of job referral centers are available that provide . . . . .

There are two things available through the guidance office, a job directory and a list of jobs. Therefore you need a plural verb, are. If you change the sentence around a bit, the error becomes more obvious.

A job directory and a list of job referral centers that provide information for students in need of employment IS available through the school’s guidance office. It’s more obvious here that “is” is not correct.
