It may refer to the story about two people going to the beach and seeing thousands and thousands of starfish washed up on the shore, dying. One of them starts throwing the fish back in the water one at a time. The other one says, “You are wasting your time. There are so many here, what difference does it make if you save a few of them?” The first one says, “It makes a big difference to the ones I save.”
So “giving up the starfish” may mean not helping one person simply because you are not able to help everyone.
